Is this the right NSW WHS SWMS?

WHS Form 05 is the Safe Work Method Statement from the Housing Industry Site Safety Pack. It documents how high risk construction work will be carried out safely on site.

Who has to complete the NSW WHS SWMS?
The PCBU, principal contractor, or supervisor preparing the SWMS for high risk construction work before the task starts.
When is it required?
Before starting high risk construction work that needs a documented safe work method under WHS requirements.
Where does the completed form go?
Keep the completed SWMS on site, make it available to workers carrying out the task, and retain a copy with the job records. Tradie Forms does not lodge the SWMS with a regulator.
